Instructions For Completing the CFIA National Import Service Centre Cover Sheet

Request for Documentation Review for Imports of Live and Raw Molluscan Shellfish

The fax cover sheet "Request for Documentation Review" should be completed as follows:

Complete sections to identify the name of the importer, the name of a contact person for the importer and their fax and telephone numbers.

The Customs Transaction Number is mandatory for all import requests.

In the case of roe-on scallops from Chile, a copy of the original Health Certificate to Export Raw Roe-on Farmed Scallop Products Destined to Canada - (Spanish only) issued by SERNAPESCA must be faxed with the Request for Documentation Review (CFIA/ACIA 5272) to the National Import Service Centre (NISC).

A copy of the original certificate must also accompany the import notification. Importers are required to keep the original certificate on file and have it available for inspectors upon request. The original certificates are not required for customs clearance of live and raw molluscan shellfish.

Identify the type and quantity of shellfish being imported in the sections "COMMODITY/PRODUITS" and "QUANTITY/QUANTITÉ"

Identify the location where the shellfish will be available for inspection in section for "Delivery Address / Adresse de livraison"

Do not complete section "MEAT SHIPMENT/EXPÉDITIONS DE VIANDE"

Do not complete section "UNMARKED MEAT SHIPMENT/EXPÉDITIONS DE VIANDE NON MARQUÉE"

Completion of the "TRANSPORTATION/TRANSPORT" section is optional and can be used to identify the means of transport (e.g., trucking company, vessel, container number, airline).

Identify the name and registration number of the shellfish shipper number (e.g. 9876 SS) and the country where the shellfish were harvested in the "COMMENTS/COMMENTAIRES" section.

No further information is needed. There will be no additional CFIA charges for documentation review of live and raw shellfish shipments as this service is included with the CFIA Fish Inspection Service Fee for imported fish products.
