- Annex A – Health Certificate for fresh meat, including minced meat, of domestic porcine animals (Sus scrofa) (POR-CA), for dispatch to the Republic of Macedonia from Canada
- Annex B – Health Certificate for meat of poultry for dispatch to the Republic of Macedonia from Canada (POU - CA)
- Annex C – Health Certificate for fresh meat, including minced meat, of domestic bovine animals (including Bison and Bubalus species and their cross-breeds) for dispatch to the Republic of Macedonia from Canada (BOV-CA)
- Annex D – Health Certificate for fresh meat, excluding minced meat, of domestic Solipeds (Equus caballus, Equus asinus and their cross-breeds) (EQU), for dispatch to the Republic of Macedonia
