  • Certificate of Origin and Hygiene (CFIA/ACIA 5003)

    For exports of geoduck clams, please add the following statement:

    "Harvest area(s): XXX, Province, Canada (not closed for PSP at time of harvest). Harvest date(s): space. These geoduck clams are acceptable for human consumption."

    XXX = DFO Area and sub area of the geoduck harvest for the consignment

    Province = enter province where harvesting of product took place
