Information required to complete the sanitary certificate CFIA/ACIA 5898 for fish oil exported from Canada to Argentina

  • To complete the certificate, it is strongly recommended exporters work closely with their importers
  • Each page of the certificate should be signed and stamped

Country of dispatch

  • The country of dispatch must be "Canada"

Part I – Details identifying the fishery products

Species (scientific name)

  • The scientific name or Latin name of the fish from which the oil is derived as defined in accordance with Argentinian legislation

State or type of processing

  • Specify the state of the fish oil at time of export or the type of processing used to make the fish oil (example: raw, refined or encapsulated fish oil)

Type of packaging

  • Identify the type of packaging according to Argentinian requirements

Lot identifier/date code

  • Provide the lot number and date code for the product

Number of packages

  • Provide the number of boxes or packages for products

Net weight

  • For products: provide the total net weight in kilograms
  • Net weight is defined as the mass of the goods themselves without immediate containers or any packaging

Temperature required during storage and transport

  • Argentina requires that shipments be maintained at a minimum temperature during transit which must be stated numerically on the certificate in degrees celsius

Part II – Provenance of the fishery products

Address(es) and/or registration number(s) of production establishment(s) authorized for export by the CFIA

  • Provide the name and address (street, city, and province/territory, as applicable) of the physical or legal person who sends the consignment
  • Provide the approval number, assigned by Argentina's competent authority
  • Provide the SFCR (Safe Food for Canadians Regulations) licence number, assigned by CFIA

Name and address of consignor

  • Name and address of the natural or legal person to whom the consignment is intended in Argentina or third country of destination in the case of transit

Part III – Destination of the fishery products

The products are to be dispatched from (place of dispatch)

  • The name, address and approval number, if required by Argentina, of the establishment from which the products come from

To (country and place of destination)

  • The place where the products are sent for final unloading
    • Give the name, address and approval number of the establishment of the place of destination, if applicable

By the following means of transport

  • Means of transport leaving the country of dispatch
  • Mode of transport: airplane, vessel, railway, road vehicle or other
  • Identification of the means of transport: for airplanes the flight number, for vessels the ship name(s), for railways the train identity and wagon number, for road transports the registration number plate with trailer number plate if applicable
  • In the case of a ferry, state the identification of the road vehicle, the registration number plate with trailer number plate if applicable, and the name of the scheduled ferry

Name of consignee and address at place of destination

  • Name and address of the natural or legal person to whom the consignment is intended in Argentina

Part IV – Attestation

  • Compliance with attestations are verified by CFIA at the time of the request for export certification
