I, the undersigned, am the person in charge of the animals intended for slaughter for human food. I declare that I have read and understood the requirements of this declaration and that the following is true and accurate.

1. No cases of anthrax have been reported from this farm / holdings during the last 20 days prior to shipping the animals for slaughter and these holdings are not subject to prohibition as a result of an outbreak of anthrax during the previous at least 20 days.
2. I keep treatment records for these animals and that I will make them available to the Canadian Food Inspection Agency (CFIA) on request.
