BSE surveillance candidate questionnaire

Bovine veterinarians, cattle producers and staff at necropsy rooms and abattoirs can now use the online questionnaire to check if a sick bovine is a candidate for BSE surveillance.

The questionnaire will take you less than 3 minutes to complete. If the sick bovine is a candidate, you will receive a unique identifier (UID) to submit a brain sample to the laboratory and to invoice the CFIA.

The personal information you provide on this questionnaire is collected for the CFIA under the authority of the Health of Animals Act for the purpose of managing the bovine spongiform encephalopathy (BSE) program and is protected in accordance with the Privacy Act.

Information may be accessible or protected as required under the provisions of the Access to Information Act or the Privacy Act and will be stored in CFIA personal information bank PPB 019.

Under the Privacy Act, you have the right to access your personal information retained by the CFIA and to request corrections to it. You also have the right to file a complaint to the Office of the Privacy Commissioner of Canada regarding our handling of your personal information.
